Dr. Lazoff is a Physiatrist practicing Physical Medicine and Rehabilitation. His specialties include occupational and sports medicine, electrodiagnosis, pain management, and spasticity management with Botulinum Toxin. A graduate of Indiana University School of Medicine, Dr. Lazoff completed his Residency at Frazier Rehabilitation Center, University of Louisville. He is Board Certified by the American Board of Physical Medicine and Rehabilitation. Dr. Lazoff is a member of the American Academy of Physical Medicine and Rehabilitation, the Association of Academic Physiatrists, the Physiatric Association of Spine, Sports and Occupational Rehabilitation, the American Academy of Disability Evaluating Physicians, and the Fort Wayne Medical Society.
